Look in MVS System Codes manual for wait state 064 where the four bytes have this format

   x0rrrwww
where:
x Irrelevant information.
rrr A hexadecimal reason code indicating the reason for the
        failure.
www The wait state code.

Under Operator Response for entry code 009 it says to re-IPL specifying a COLD start at the IEA101A enter system parameters prompt.
