Your set-up starts-out very similar to my latest cabinet, but differs on
the video card, the card's s-video output, and the use of a fixed
frequency monitor...unfortunately, I think that list of differences
pretty well sums-up where you're having problems.

I didn't see any documentation in my kernel sources on the rivafb
driver, but is there possibly an option in that driver for enabling
s-video output? [I know an old Voodoo card I tried a few years ago was
really picky about outputting an s-video signal]

Since you're using a fixed-frequency monitor, you're not really going to
gain anything from a card-specific fb driver...but, you're right,
advmame fb video won't work with the vesafb drivers. You should be able
to use SDL compiled with fbcon support [default] with the vesafb or
vesa-tng driver and still avoid running X.

It turns out advancemame is my daddy, as I am
completely unable to get 
it in linux.

I am running gentoo with 2.6.8, without X.  I have a
geforce2 mx 400 and a 27" toshiba TV NTSC using
s-video.  At first I tried getting it to work with
svgalib, but svgalib wasn't so inclined.  I edited
...
