Thanks everyone for your feedback.  I think I might try one of the
earlier ArcadeVGA cards if I can find one.  I'll let you guys know how
it turns out.

I have used both a geforce 4 and a ATI radeon 7000/VE (my current card) and
 I"ve had the same difficulty with both, unable to hit low pclock values.
 Setting it higher to 7 or 8 MHz however gave me a very usable range on my 15Khz
 Standard Res monitor. 
 
 If I may recommend, although I have no real experence, I think the AdvanceVGA
 card from Ultimarc is maybe the best card to use for a 15KHz or a MultiSync
 arcade monitor. 15KHz out of the box, a software patch to add (not change to)
 30 KHz, and Advancemame will overwrite it all to give you one of the better
 RAMDACs for low pclocks which equals low resolutions. It"s definately going to
 be in my next arcade cab with a Betson Multisync. It should definately be
 something that you give a serious look at. Unfortunately though, it"s a little
 more expensive then most of the others used for AdvanceMAME cabinets at $80 US,
 but it still worth a good look.

 Please note that the most recent ArcadeVGA cards are based on the
 Radeon 9200 chipset instead of the older 7000 chipset.
 
 This chipset is probably not well supported by the DOS version of
 AdvanceMAME. I don"t know for sure because I don"t have a such board.
