I've wondered about this, too. My assumption is that the actual cost of running
a planet has to do with it's productivity, which is connected to population.
When the pop is super low, I think the results can be like this. I'd be
interested in hearing a real answer, though.

This sounds right to me.

Most likely. We've been told that "practically everything" is subject to it.

That's all a question of strategy. I crank it up when I want to strip-mine an
unprofitable planet, or one that's in danger of being overrun. I also raise it
somewhat to terraform a planet which I think can be profitable in a short
number of turns (less than 10, on a 10 years/turn scale).

My impression is that they're fairly constant. They look a bigger at the
beginning of the game than later on, when you've got more income.
