Merged, many thanks.

I merged your bootstrap patch with the upstream bootstrap project, and then
updated the M4 files from the latest bootstrap master.  I also edited some
of your other changes a little before applying.  Let me know if any of them
need additional work to maintain compatibility with your OS/2 system.
