Build problems with current tree (doxygen)

Building the documentation with doxygen is enabled by default in 
options.mk and doesn't seem to work on all versions of doxygen (1.2.18 
shipping with cygwin for example).

Here are the errors from 1.2.18:

On linux, I get this:

Error: tag HTML_HEADER: header file `header.html' does not exist

I haven't used doxygen in a long time, so I'm not sure off the top of my 
head how to fix this.  For now, I'm going to change options.mk to not 
use doxygen by default.
