Actually jorrit that's why gamers despise optimus while heavy mobile
users and casual gamers love it. It's unreliable also on windows
that's the main issue of it (aside from the performance loss related
to the technology design itself). The things you describe is the
normal workflow of any user of optimus even with commercial games, so
I'd not be sure the blame is of cs. Anyway default should let the
system choose what it regards as the best approach and it might even
not be nvidia, but for sure the gdi thing seems a fault in the driver
itself IMHO.
I'd say to check if there are more up to date drivers from some oem
with similar setup (i'd check laptopvideo2go if they have something)
