One of the "bunch of other stuff" or the phone's own sound settings may be the culprit.  Check the Preferences screen for the phone.  Under Sounds and Alerts, check the settings for Application - calendar.  Under Tones you can set the alarm to sound once or multiple times - perhaps that got changed.

Or Butler?  Some other app that can handle alarms?  

Finally, perhaps it is simply that somehow the Preference/Alarms in DB6 got "Datebk6 Handles Alarms" unchecked...in which case my suggestions above can be skipped - just check the box again!
