In my experience, "accidentally sent directly" is a FAR more common 
issue.  I also really hate it when I get multiple copies of some email 
because one is a To and one is a CC, which is what usually happens.

I know, old argument, but I'm decidedly on the "Reply-To is good" side, 
FWIW. :-)
