My first Mac Dvorak keymap (back when I rolled my own) left the  
command key combinations in their QWERTY positions. This is still an  
option on the Mac. Eventually I decided to remap those as well. I  
tend to think the letter for unfamiliar command key combinations, and  
since I use lots of different programs something's always unfamiliar.

In practice, I find typing command-C or command-V using _both_ hands  
isn't much harder than typing Shift-C and Shift-V that way. It is a  
nuisance when I miss on a paste and accidentally hit command-W (close  
window), but the damage usually isn't fatal.

I use my thumb to hit the command key, so on a PC it would be a bit  
harder. I suppose I could learn to hit Ctrl with my pinky if I really  
applied myself.
