Correction: Entity (&) problem in attributes (should be elements).

Hello again,

My previous message stated that the problem with &amp; occurred in 
attributes.  I was incorrect.  This problem occurs in #PCDATA in 
elements.  The ampersand (&amp;) is handled correctly in attributes.  
Sorry for the confusion.  Would still appreciate help.  Thanks.
