InsertPngFileCommand

Forgive me if this has been covered.

InsertPngFileCommand (or any other command for that matter) takes in a
Context object.  Where does this context object come from.  Is it
automagically created or do I have to manually create it and set the
properties?  Which brings me to my next question -- where does this
command get the name of the png file to insert?
