At the top of the file I have:

[i]<include file="webwork-default.xml"/>[/i]

This is the only action that I have found where this happens. Recently we did an upgrade from webwork 1. Everything works great accept for this action. There are similar actions to this one which almost do the same thing and I have verified this works.
