My understanding was only when the indexes got corrupt, that is when you
notice that the search functionality is empty or stale.
Or after you have deleted the index (eg you've moved the scarab instance
from one server to another).

It is normally incrementally updated.

Not AFAIK.
But it shouldn't be so difficult to achieve scripting it (using lynx or
wget, and getting around the login process and posting the final "Update
search index" request.
