How to build Interactive Voice Response (IVR)?

I'm currently developing an application which integrate some media like :
SMS (short message service), E-mail,Faximile and Telephon.
This application hopely can manage data which come from those media and
save them into one database.These data has any format based on the media,
such as : text(from SMS, E-mail), image(from faximile -->*.tiff) and
voice/sound(from telephon).

Actually,I build an interactive voice response (IVR)system which based on
telephony.So,basically,it has two main problem :
1. How to receive incoming call(include senderinformation) ,and
2. How to Make outgoing call (ex: playing audio file when caller press any
button on telephon,make a call).
Anybody know how to solve these problem?
Could you send me any example code ?please..help me..
