Problem, I described, I have only with external (outgoing) calls.
I think that it's depend on progress indicators in ISDN setup messages.
We tried to work with on gateways and sometimes we resolved the problem.
But not in all cases. And sometimes we get it worster than before :-(

RTP stream is started before because you are using H.323 fast connect.
So voice channel is established before real connect.

Currently we have no developer contract.
