to me there are many things that should need to be done, and most of those are 
really small.
one big factor we lack is the "first impression" factor, what destroys this is 
a lot of small annoyances, like icons or layouts misaligned by one pixel, text 
not prefectly vertically centered, to many fonts in a single screen etc.

all those factors contribute in augmenting the visual noise, and all of them 
individually are almost invisible, but add up very badly one on top of each 
other.
it's also hard because most of our developers (well, most technical people in 
general, not only developers) have a complete blind spot on this, in part for 
proximity blindness, in part for just not being wired this way. So, not easy 
to do, but let's do it.
