phpLDAPadmin 1.2.0.3
openldap2-2.3.32-0.27
apache2-2.2.3-16.21
php5-5.2.5-9.20
SUSE Linux Enterprise 10

Install 1.2.0.3, copy config.php.example to config.php,
open config.php and remove the # in the three lines with
$config->custom->debug

When invoking the pla directory via web browser it shows:

Fatal error: Call to undefined function syslog_notice() in /srv/www/phpldapadmin-1.2.0.3/lib/functions.php on line 567

Inserting debug_print_backtrace() in functions.php line 3054 adds:
