612 is IFD_COMMUNICATION_ERROR.

My CCID has a lot of usefull debug messages. I don't know if it is
possible to see them using libchipcard.
Does libchipcard implements log_msg() and log_xxd()?

Do you call IFDHSetProtocolParameters() with the correct parameters?
The Protocol flags is really important. The IFDHSetProtocolParameters()
function set the reader and the driver in the correct configuration.
