Mathematical accents

Which is the best way to give mathematical accents (in presentation
markup)? The MathML spec seems to try to avoid combining marks
whereever possible (well, at least most of the are not clickable in
MathML's character tables).
For example, if I want to print $\ddot x$, I can use a combining
diacritical mark, or I can use <mover> with a spacing dieresis. Is
both correct? For some letters, the composed character is already
available. Is using this totally equivalent to the <mover> variant?
For the hat accent, I may use two forms:
^ U0005E, CIRCUMFLEX ACCENT , Hat
ˆ U002C6, MODIFIER LETTER CIRCUMFLEX ACCENT , circ
(I may even use the locical "and" operator.) Is one of these
canonical or are all equivalent within an <mover>?
