In practice, when a SHOULD is violated, the authors often did not 
carefully weight the implications, but didn't consider the RFC or impact 
at all.

In that case, the browser user is usually a company employee or ISP 
customer (which maintain the proxy) and often gets the browser from 
there. The default prefs can then be adjusted appropriately.
