Loading images

When I hit the image button to upload a picture, sometimes I get a box
with OK, CANCEL & HELP (and nothing else) (When I try help it crashes
netscape). At other times I get nothing.  The Javascript error is

Is there somewhere I need to define 'InitEditorShell' ??
