Are you working with widescreen video, by any chance?

DVD-Video allows for up to 36 buttons and up to 3 button groups.
However, it appears the button groups are used for 2 conflicting
purposes: with 4:3 video, it lets buttons in different groups use
different colour tables; but with 16:9 video, the groups have to be
used to specify different button layouts for use on a) widescreen sets,
b) narrowscreen sets with pan-and-scan, and c) narrowscreen sets with
letterboxing of the video footage.

In the latter case, dvdauthor requires each group to have the same
number of buttons, so the limit in each group is 36 ÷ 3 = 12.
