Originally, the placeholders for the VOBUs, NAV PACKs etc must already
have been inserted by whatever did the MPEG generation/conversion.
dvdauthor would just fill in the information in the spaces already
provided.

However, back in February 2010, I added Martin Crossley’s ivtv patch,
which will automatically generate VOBUs if the MPEG file didn’t define
any. It makes a new VOBU every time it sees an I-frame.

Of course, this behaviour is suppressed if the MPEG file already has
VOBUs. In any case, the message “fixing VOBUs” just means it’s filling
in the final VOBU information, not that it’s generating them—there’s a
separate message for that.
