Previous versions of the DVDAuthor suite had inconsistent defaults
regarding the video format: the dvdauthor tool itself used NTSC unless
otherwise specified, while spumux fell back to settings more appropriate
for PAL.

So I reworked it all so that you could set the default for all tools
(in order of increasing priority) at build time, by systemwide
configuration, per-user configuration, or in the XML control file.
Rather than play favourites, the build default (unless otherwise
specified) is to have no default format at all, so you get an error if
the video format is not properly specified on any invocation.

Unfortunately, this is the result of a bug in 0.7.0. The specification
of the video format attribute in the XML control file cannot be
positioned high enough to apply to the whole file, but it is needed for
certain fields in the VMG IFO even though there might be no video in
the VMGM. I fixed this in a later patch. Maybe I should do an 0.7.0.1
release to get this out there...
