I would say, yes, you can take over maintenance. You could host a new copy somewhere and make your changes available as per the GPL in the original project. The code is already out, so asking the original author is a good idea but not a hard requirement IMO.

We have done this already for some externals which live in https://github.com/pd-externals. If you want, the repo could be included here with you as the project maintainer, etc.
