Some conjectures and bald-faced assertions:

By the time the leader emerges, the leader has leadership skills. 
(Otherwise, the person wouldn't have emerged as leader.)  If someone
on the team has leadership skills, I'd expect that to influence how
the emergence happens.

Everyone on every team has at least a tiny hint of leadership skills.
 These skills evolve along with the team, and in the direction of the
team's needs.  Given that, I think an effective leader can emerge from
many teams whose members start out with no particular leadership skills.

Maybe the "higher needs" emerge in accordance with the choice of
leaders (among other factors).  It isn't clear to me that "the higher
needs of the organization" exist prior to the emergence of leadership.

> (3) At what scale does this emergent leadership work?

My reading of history suggests that it works at all scales.  Every
appointed leader was appointed by some other leader.  If you trace the
appointments far enough, you necessarily reach an emergent leader.

New rule I just made up:  Every appointed leader derives authority
from an emergent leader who has greater authority.  (I think this
relates to another principle that someone wiser than me articulated: 
All authority is granted from below.)
