The hubris of the people in charge is one thing that has not changed in the
past 20 years.  We asked this same question 20 years ago, and will be 20
years from now, too.

Face it, software development is primarily a people problem - not just the
people developing the software, but also the customers, subject matter
experts, managers and ultimate decision makers.  Scrum and XP works well,
is large part because they provide more compensation for human error, but
they do not address the natural resistance of those in control losing some
of that control.

The emergence of SAFe (which in practice kills agility by ceding control to
people who are neither developing nor consuming the software) is prime
evidence the human resistance to maintain the status quo is the answer to
these 5 whys.
