My alternative is along the lines of Participatory Design, where the
stakeholders don't sign off the requirements, they sign on. That is,
they are involved day-to-day in the process. Executives won't be, at
least not often, but the people whose lives are most deeply touched by
the system are there in the room.

I've been thinking about this some more, and I think Sony are a good 
example of what can be done with product design. Sony produce new 
hardware products in almost the same way software is designed and 
developed.

I've heard stories that Sony's product development team can turn an 
idea into a new product on the shelves in a week.

I don't know how true this is, but if they can do it that quickly, 
it's because they make incremental enhancements to a highly modular 
design, not sweeping big changes. They will sell a small number in 
markets like Akihabara, with phenomenal stock turnover in the shops, 
and if a product is a hit, then they will launch it nationwide or 
worldwide.
