Maybe you need to stress that link more, then.  i.e., that your book 
builds on - and assumes a good prior working understanding of - TDD. 
Maybe even go into a section (or even a short chapter) where you review 
some basic TDD concepts.  Otherwise perhaps a non-test-driven reader is 
missing sufficient context to grok the book properly.

Don't let this ruffle you.  Try to think of this as constructive 
criticism that can only help the book's readability ... and eventual 
success.
