Summary: Add Email variable to gravatarplugins default image url

Initial Comment:
In order to allow the md5 hash of the comment submitters emailaddress to be used in the default image url passed to gravatar, I created an %email% var with gets substituted in the gravatar default url prior to appending to the image url. I also had to url-encode the resulting url in order to allow any request parameters be passed to the default url. The resulting patch is appended.

Can you explain why the md5 hash of a submitters email is useful in the
'default' URL variable? The submitters email should only be used for the
"real" avatar, not the default avatar?

I mean, shouldn't the reason for a default-avatar be that it shows a
static avatar?

Maybe I'm just not understanding things, so I'd love if you could lighten
me up :)

Best regards,
Garvin
