Always index in 6b3

Hi Jens,

I've moved to 6b for a while, because it's quite stable now. But it produces 
indexes always, even when these indexes are buggy. So even when 
authorformat=indexed is not given, makeindex picks up the authors' names in 
the index. Could you solve that issue. Otherwise, I'll need to return to 5p 
and natoptargorder, because I've change all my optional arguments already.
