Thanks for all your help. I finally got it to work...but have an
interesting observation.
Using the \usepackage[multiple]{footmisc} does work, as was the
original and most recent suggestion (thanks to you both).
BUT...it doesn't work if placed right after the \usepackage{jurabib}
statement in the preamble.
As examples, when I use the following preamble:

\usepackage{babel}
\usepackage[titleformat=italic,titleformat=commasep,commabeforerest,oxford]{jurabib}
\usepackage{esint,epsfig,graphics,latexsym,amsfonts,amssymb,verbatim,
setspace}
\usepackage[multiple]{footmisc}
\usepackage{index}
I DO get commas separating my footnote superscripts. [sidenote:
However, they are strangely formatted, (i.e. 4 ,5 ,6), with a numeral,
then space then a comma then the next numeral without a space between
the comma. Thoughts on this formatting?]
However, if I use the preamble

Seems strange to me. But I thought I'd add the information to the
list. I certainly don't know enough about the compiling process to
see why I'd have a conflict between the two coded preambles. Perhaps
someone does though, and can clarify the strange comma/space issue in
the superscript formatting?
