Aha!  I was wondering about the changes.  I was creating the .texi by
hand and it is *very* tricky to get right.  Your additions to the
index and cross-references are very welcome.

I like this.  It should be a section (not appendix ... because the
index is a section) with subsections to help separate the commands.
Alphabetize within each subsection.

I don't know about commenting on each command.  For example, the fonts
commands are all pretty well supported.  As are the logos and sectioning
commands. Special characters are handled pretty well except in  
equations.
The *mode commands tend to be pretty good.  Most of the uncategorized
work well.  Many of the preamble commands are just there to improve  
parsing.

On the other hand, I don't thing that I want to document each  
command.  I
can certainly help with some, but honestly, it looks really tedious ;-)
