There is also an extraneous comma and trailing space after the 
semi-colon.  I think it should repeat chap. 3 as it does; Allen and 
Carifio are the defaults, then, and the year/chapter is added on for 
the individual cases.  This cite style looks like something from 
Bizarro world anyway as far as I'm concerned :).

The tabbing one was a long math mode expression, and I couldn't figure 
out what was going on in tabular.
