The author of winedt analyzed the problem in october 2010. As far as
I understood the problem arises when pdflatex doesn't run inside the
standard windows console but in a console of the editor:

"The problem with on-demand package installation and the default
MiKTeX option "ask me first" is that MiKTeX's accessories assume
that they are running in Windows Console and thus pass an "invalid
[non-window or hidden window] handle" when they try to create a
dialog to confirm package installation. This happens if console is
hidden or detached and STDHandles and pipes are used to capture
output and input -- which is the case when WinEdt executes these
applications in its own console. "

I don't know if this counts as a bug in miktex. I actually don't
care: I have set up winedt so that I always use the standard windows
console. Also the winedt author found within three days a way around
the problem for the people which wants to use the internal console
of winedt. I don't know if the problem affects other editors, at
least I have never heard complaints from emacs, texworks etc users.
Only texniccenter users are running constantly in this trap.
