Then it is no really senseful to install a multiuser installation of
miktex. Install miktex in your account in single mode and you will
not have to struggle with the admin + user mode.

This can happen easily, e.g. after a on-the-fly installation or if
you used lualatex as user and miktex recreated for some reason the
format.

Yes. User settings always wins over admin defaults. You had a user
format and you must update it as user. If you don't know if the user
or the admin should do some task, then do simply both: Run updates
as user + admin, update FNDB as user + admin and create formats.
Then you will be on the safe side.
