Do you literally mean the icons or the buttons too? And are these specifically MiKTeX buttons which are not scaling or other buttons too?

Either way, welcome to 4K hell! It means that either/both of Emacs or the some MiKTeX assets are not scaled for high resolution displays. It's slowly getting better with updates to various pieces of software (Windows 10 itself included) - though it's necessary here to determine if you're dealing with a bug in MiKTeX or Emacs.

PS If it's any consolation, it's even worse when you have a multiple monitor setup with a mix of 4K (3840x2160 UHD) and FHD!
