This was fixed by 1.11.6 & 1.12.1.  If you can't upgrade, you might try 
`cvs rm -f ./--and-hyphenation.html'.  I managed to get both to work 
with 1.12.1 from here, but there were two files with that name and I 
wasn't sure you wanted them both removed.
