Is this happening to all the commands?
What happens with NOPASSWD (just in case)?
Isn't sudo strace command also an option to further debug?

Strace sudo will not work, but this should help if the problem is on the command.

Also, at least to replicate the problem, ++ on using Todd's binaries.
