I've created new IDs for the ITV1 macro-regions being used for the
ITV1 +1 and ITV1 HD services. The old IDs for these services will be
kept active for a while to allow users to migrate to the new IDs, but
will be removed to avoid confusion and aid future maintenance.

The new IDs for ITV1 +1 services are:

London: tsod.plus-1.london-macro.itv1.itv.co.uk
Central: tsod.plus-1.central-macro.itv1.itv.co.uk
Granada/Border: tsod.plus-1.northwest-macro.itv1.itv.co.uk
Meridian/Anglia: tsod.plus-1.southeast-macro.itv1.itv.co.uk
Yorkshire/Tyne Tees: tsod.plus-1.northeast-macro.itv1.itv.co.uk
West/Westcountry/Wales: tsod.plus-1.west-macro.itv1.itv.co.uk
STV/Scottish Border: tsod.plus-1.scotland-macro.stv.tv
UTV: tsod.plus-1.ni-macro.u.tv

The new IDs for ITV1 HD services are:

London: hd.london-macro.itv1.itv.co.uk
Central: hd.central-macro.itv1.itv.co.uk

Apologies for making these changes so soon after adding support for
these channels (and especially to ITV1 +1 West users) - it should make
things easier to update and maintain in the future.
