I'm not so sure about this. Quite a few shows get relaunched after a few years break with a new presenter, and/or on a new channel (University Challenge, or the newer Bullseye with Dave Spikey) but they keep the old title, which I think is the correct thing to do. Even Room 101, which after a five year break came back with not only a new presenter but a new format, kept the same title.
