Creating a repository is done once and for all, on the server side. I think TortoiseCVS does not provide any dialog to actually initialize a repository. Anyway, it's not hard; just download CVSNT and follow this guide:

No. Use an "alias" name. Suppose you want your repository located at d:\cvsrepos\myrepo on the server [it doesn't matter if "the server" is actually your own PC]. From the CVSNT Control Panel, you initialize that
repository and give it a name like /myrepo (as explained in the Guide above). Then, in TortoiseCVS dialog, use /myrepo as the repository folder.

Better leave that blank instead.

You don't - sspi will use your current Windows credentials.
