Not sure about a tutorial, but the recommended practice is to make a
series of armatures for a joint rather than just a single joint. For
example, instead of an upper arm armature and a forearm armature, with
the rotation point where they connect, make one or more "elbow"
armatures that will bend in a curve and create a distance between the
interior faces of the upper arm mesh and the interior faces of the
forearm mesh ("interior" being the anatomical definition, not "inside"
the mesh).
