Have you looked in the skinning chapter of the book (or wikibook for
that matter)?

 

You can select which vertices are influenced by an armature when bending
a joint. Seems to me you aren't using enough, or you don't have enough
vertices in the neighbourhood to allow for a smooth bend.

I've created a figure in Blender:

The arms pinch off like a straw when bent.  Any clues as to a tutorial
which shows me how to avoid this?
