As it turns out the correct patch for this is fairly well known; it
requires a complete set of backward-compatibility #defines for Apache,
but it hasn't been applied even to the latest CVS version of FastCGI.
I suggest you get it in there as a backward-compatibility hack while
you decide what else to do, and tell Apache2.2 users on the webpage
that they need the CVS version.
