  Add ORM::unloaded_instance() that gives us a new version of the current
model but preserves whatever other database operations were in the chain.

  Reload the root item in test_i_can_edit() so that we get a fresh
related AccessCache model after adding the new group.  Now that we're
using relationships which cache their results, we'll have to do more
reloading.

All Access_Test tests now pass!
