StopSpamForum works great. I actually did a version of the G2 captcha  
module that uses it when I started getting spam through the contact  
page.

A similar concept is the HttpBL from Project Honeypot. There is a  
drupal module including for D5: http://drupal.org/project/httpbl/ .  
Using the SMF implementation together with some homebrew stuff,  
haven't had a spammer register since on a pretty moribund site ... the  
sort that usually gets overrun.

PErhaps installing an existing module might be less of a task.
