Formo code is missing preambles

Hey Shad - the File_Structure_Test almost passes.  There are a few issues,
though and one of them is Formo.  It looks like many of the transparent
extension classes are missing the security preamble at the top.  Can you
fix that?  I'll work on fixing it up in all the other modules.  Just run
the test to see what's broken.
