Oh weird.  I thought that users would rather explicitly enable plugins
rather than implicitly enable them.  Interesting.

I think this is actually really complex and difficult to do because it
requires us to re-order the items in a callback list either during
construction or after the list has been built.

I agree we should defer this issue until we discover a solution is
really needed.  If it turns out it is really needed, we'll need a more
general solution that can be applied to all callback lists--not just
the entrycache ones.
